He is a genius and is in the A-class in college, the class where the cream of the crop are in. Not only that, he is also always the top in school, sometimes getting full marks in the exams. In fact, he has an IQ of 180! He later went to the Faculty of Sciences [I'm not sure about this] in university. He is the most, most popular guy in school, for he has all the qualities girls look out for - he is intelligent, good-looking, good in sports (esp. tennis), the son of a rich father, etc. The reason why he is always so cold and aloof is actually because he was once dressed up as a girl by his mother when he was young. From then on, he says that he doesn't like stupid people.
He was, however, really in love with Kotoko as well, as can be perceived from the subtle signs of love he shows for Kotoko - including mild jealousy, because Kotoko did have some admirers ^^;; such as Kinnosuke and Takendo and later Keita (her nursing friend). When he found out that Kinnosuke had proposed to Kotoko, he waited for her the rain ^.^ and proposed to her! Hardly anyone would believe that, even Kotoko herself. And Kotoko didn't turn out to be a good wife (as in the stereotype of a good wife). Later, in the manga, Naoki said that what was attractive about Kotoko was her persistence. Naoki became a doctor eventually - and was a very capable one, even when he was a trainee doctor. As can be imagined, patients, nurses and even doctors fell in love with him. Irie
Naoki is the other main character of the manga, and the love of Kotoko. 
Like
many other girls, Kotoko fell deeply in love with him. However, he was
indifferent to her just as much as he was to all the other girls, though he
did appear a little closer to Reiko, his university classmate.
